By applying for and receiving electric service from Paulding Putnam the applicant becomes a member of the cooperative. Members agree to follow the cooperative’s Terms and Conditions of Service, as outlined in the Code of Regulations, and to pay the rate established by the Board of Trustees.

- If the form is not returned within 30 days, service may be scheduled for immediate disconnection.
